“Thank you for considering Northern Assist Care as a disability service provider. It means a lot that you’re exploring how we can be part of your journey. Our goal is to provide personalised, compassionate care that empowers you to live life your way - both at home and in the community. We can’t wait to speak with you!”
Kylie Goss - Managing Director & Founder of Northern Assist Care In Home Support

Once we receive your referral form, our dedicated Client Services Team will contact you to discuss suitable services and answer any initial questions you may have. If you decide to proceed, they will arrange a face-to-face meeting to begin the onboarding process.
We encourage you to involve family members, friends, and/or your Support Coordinator during this stage, as we believe a collaborative approach ensures the best outcomes in planning and delivering your supports.

Following your initial meeting, our Client Services team will prepare your Service Agreement, Support Plan, and Schedule of Supports based on the information you have provided. These documents, along with any other relevant forms, will be reviewed and signed during a final meeting before your supports begin.
During this meeting, our Client Services team will ensure you fully understand your rights and responsibilities, as well as those of our organisation. They will take the time to go through all documents in detail and answer any questions you may have.
Once all documentation is finalised, your supports will be scheduled to commence on an agreed start date, in consultation with our Rostering Team.
If you’d like to speak with someone before submitting a referral form, please contact client services on 08 8102 4232. We would love to hear from you!
